After 32 years of serving this community, I have made the difficult decision to retire and close my medical practice, effective September 30, 2025. This decision has not been easy, as it has been an extraordinary privilege to care for you and your families over the years.

I am deeply grateful for the trust you have placed in me as your physician. Your stories, resilience, and shared moments have enriched my life and made this journey profoundly meaningful. It has been an honor to be part of your healthcare and to witness the strength of this community.

To ensure a smooth transition, I encourage you to begin arranging for your ongoing care with another healthcare provider. Your medical records are confidential and will remain secure. You may request a copy of your records or have them transferred to your new provider by contacting idahodiabetes@protonmail.com before September 30, 2025. Our staff will assist you with this process.

For help finding a new physician, you may contact your insurance provider, local medical societies, or nearby healthcare facilities. I am confident you will find a provider who will continue to deliver the compassionate care you deserve.

For patients with an outstanding balance or those on a payment plan, please note that all final payments must be received by September 30, 2025. You can make payments by mailing in payment to the above address, calling 208-389-2213 or in person at the office. If you have questions about your account, please contact our billing manager, Nicole, at 208-389-2213 as soon as possible.
